As we age, we face various challenges regarding our health and safety. We want to be able to stay in our own homes for as long as possible. As children of aging loved ones, we want to be available to care for or make sure our loved ones are safe. In today's world, family may be spread out across the state or even the country, and we can’t be there as we would like. That’s where Carolina Concierge Nursing can be beneficial. Our Private In-Home Senior Nursing Services include:

  • Wellness check-ins to include vital sign monitoring

  • Medication monitoring to include setting up a pill reminder and education about medications as needed

  • Skin assessment to aid in preventing any possible skin breakdown or infection

  • Home safety review to assist in the prevention of possible falls or other accidents

  • Assistance with contacting the Primary Physician if needed

  • Keeping in touch with family members to keep everyone informed

Additional services available for a fee include:

  • Accompanying the client to a doctor's appointment with advanced notice

  • Obtaining blood work and delivering to the lab with results going directly to the physician's office

  • Providing injections such as B12 or medications for osteoarthritis

  • Wound care with a physician's order

Getting to Know You

3-hour visit

  • Initial introduction with your nurse

  • Complete paperwork

  • Discuss healthcare needs and goals

  • Assessments including home safety, vital signs, medications and skin

  • Establish plan of care

Silver Package

1 month of weekly 1-hour visits

  • Assessments including home safety, medications and skin

  • Nutrition & Hydration review

  • Vital Sign monitoring

  • Basic wound care if needed

  • Injections (B12, osteo, etc.)

  • Monthly review of plan of care

Premium Package

3 months of weekly 1-hour visits

  • Assessments including home safety, medications and skin

  • Nutrition & Hydration review

  • Vital sign monitoring

  • Basic wound care if needed

  • Injections (B12, osteo, etc.)

  • Monthly review of plan of care

Elite Package

6 months of weekly 1-hour visits

Additional Nursing Services
For Seniors

MD Office Visits

With or without a package subscription
 

  • Accompany to doctor visit with transportation to and from home

  • Notes from MD visit and communication with a family member with client’s approval

  • Client must be able to transfer into and out of vehicle

Lab Work

With or without a package subscription

  • Blood work collected at client’s home

  • Specimen properly stored and delivered to lab facility

  • Lab will bill client’s insurance

  • Results sent to MD office

  • Must have MD order

Injections

Included in package subscriptions.

Additional charge if you do not have a package.

 

  • Supplies for injection and medication obtained from pharmacy by client

Wound Care

May be included in a package subscription. Additional charge if you do not have a package.
